APPLY

The first step to secure housing requires the completion of an application.  Applications can be picked up in the box outside the main office, mailed to you by calling 814-676-5764 Option 2. or by clicking on this PDF copy. This pdf copy can be filled out printed, signed, and submitted by mail to 110 Moran Street, Oil City, PA 16301, along with supporting documents (see below), or dropped off anytime using the 24-hour drop slot by the main office door.

Copies of the following documents must be provided with the application:  Social Security cards for all household members, photo identification for all members over 18 years of age, proof of all sources of income/assets for each household member, such as a Social Security award letter, six most current pay stubs, six most current bank statements, etc.  Copies can be made of these documents at our office when dropping of the application or, if copies can be provided, the application along with supporting documents can be submitted by mail to 110 Moran Street, Oil City, PA  16301 or anytime using the 24-hours drop slot by the main office door.

Upon receipt of the application, all information will be verified and eligibility determined.  A current phone number, email and address are necessary to complete the application process.  Notification letters of acceptance or denial will be mailed to all applicants and offers of units will be made by phone call, email or mail when units are available.
